I, , hereby acknowledge that I have voluntarily chosen to participate in physical training, exercise, and fitness-related activities at Fitness Education & Training Center LLC (“FETC”), whether such activities are coach-guided, instructor-led, semi-private, small group, open training, or otherwise conducted within the facility.

I understand that participation may occur with Fitness Education & Training Center LLC owners, employees, coaches, contractors, guest coaches, independent trainers, or other individuals operating within the facility, and that such participation is voluntary.

I acknowledge that engaging in physical exercise and training programs involves inherent risks, and I willingly accept and assume those risks.

Informed Consent

1. I understand that participation in fitness activities at FETC may include, but is not limited to, cardiovascular exercise, strength training, Olympic lifting, conditioning, flexibility, mobility work, use of equipment, and other physical activities that may result in injury, discomfort, or illness.
2. I acknowledge that coaching, instruction, or supervision may be provided by FETC staff or other trainers operating within the facility, and that I am responsible for following instructions and exercising within my own limits.
3. I understand that FETC may collect health or medical history information to assist in programming, but I remain solely responsible for monitoring my condition and limitations.
4. I agree to promptly inform any coach, trainer, or instructor of changes to my health or physical condition that may affect my ability to safely participate.
5. I acknowledge that results are not guaranteed and vary based on individual effort, consistency, and health status.

Assumption of Risk

1. I acknowledge that participation in fitness and training activities involves inherent risks, including but not limited to muscle strains, sprains, fractures, cardiovascular events, equipment-related injuries, and other physical or medical conditions.
2. I voluntarily assume full responsibility for any and all risks, injuries, accidents, or illnesses that may occur while participating in activities at FETC or while present in the facility, regardless of whether such activities are supervised.
3. I agree to hold harmless and release Fitness Education & Training Center LLC, its owners, employees, coaches, contractors, independent trainers, guest trainers, affiliates, landlords, and facility operators from any and all claims arising from my participation or presence in the facility.

Medical Clearance

1. I confirm that I am physically and medically able to participate in fitness activities and have consulted with a physician if necessary.
2. I understand that coaches, trainers, and staff at FETC are not licensed medical professionals and do not provide medical advice, diagnosis, or treatment. Participation is not a substitute for professional medical care.

General Liability Waiver & Release

In consideration of being permitted to participate in any training, exercise, or fitness-related activities within the FETC facility, I hereby waive, release, and discharge Fitness Education & Training Center LLC and its owners, employees, coaches, contractors, independent trainers, guest trainers, affiliates, and facility representatives from any and all liability, claims, demands, actions, or causes of action arising out of or related to my participation or presence in the facility, whether caused by negligence or otherwise, to the fullest extent permitted by law.

This waiver applies to all activities conducted within the facility, regardless of whether I am training under direct supervision.

Acknowledgment & Binding Agreement

I have read and fully understand this Informed Consent and Liability Waiver. I voluntarily sign this document and acknowledge that it is intended to be legally binding upon myself, my heirs, executors, administrators, and assigns.

Photo, Video & Media Authorization (Optional)

I understand that Fitness Education & Training Center LLC (“FETC”) may take photographs and/or video recordings of participants during training sessions, events, or normal facility operations. These images or recordings may include my likeness, voice, or performance.

I hereby grant permission to FETC, its owners, employees, coaches, contractors, and authorized representatives to use, reproduce, publish, and distribute such photographs or video recordings for marketing, promotional, educational, and social media purposes, including but not limited to websites, social media platforms, print materials, and digital advertisements.

I understand that:

• Participation in photos or videos is voluntary

• I will receive no compensation for the use of such media

FETC retains ownership of all images and recordings. My name or identifying information will not be used unless I provide additional consent.

This authorization applies to all media captured within the FETC facility and remains in effect unless revoked in writing.
